Medical Home and Well-Child Check-Ups
Medical Home
A Medical Home is the one place you can take your child for all your child's health needs such as check-ups, sick visits, immunizations (shots), accidents or special health needs. A Medical Home can be a doctor's office, a community clinic or a local health department. It is where the staff knows you, your child, and your child's health history. You can call your Medical Home during office hours and after-hours.
Well Child Check-Ups
Keep your child healthy by taking him to the Medical Home for regular well child exams or “check-ups.” At each visit the doctor will weigh and measure your child to make sure he is growing at a healthy rate, check his eyesight and hearing, and talk to you about his behavior. These visits also give you a chance to ask any questions you might have about your child's development.
A well-child exam or “check-up” should occur at each of the following ages:
- Infant
- Within the first month
- 2 months
- 4 months
- 6 months
- 9 months or 15 months
- Toddler
- 1 year
- 18 months
- Preschool
- 2 years
- 3 years
- 4 years
- 5 years
